What should be happening?
Our approach
SCOPE connects what should happen, what the evidence shows, what needs action and whether that action changed the position.
The SCOPE method
Each lens asks a clear question. Together they connect the records, the clinical picture and the work that follows.
What should be happening?
Are the agreed requirements and records in place?
What risk, gap or need is being missed?
What actions and priorities are required?
Did the action occur, and what still needs monitoring?
From review to follow-through
Four connected stages keep the work clear. The scope determines what is included at each stage.
Bring the available records, assessments and requirements together. Identify gaps and current risks.
What is known, and what needs clarification?
Explain the findings. Set priorities and agree the work, who is responsible and when to escalate.
A defined scope and clear actions.
Carry out the agreed review, correction or clinical support. Share practical guidance where needed.
The agreed work and documented next actions.
Confirm what happened. Review the identified risks and decide what needs reassessment or continued monitoring.
Follow-up and the next review point.
Where further work is needed, return to the review.
Evidence
Missing information stays visible. Recommendations follow what the evidence supports. Work outside current competence is escalated.
Accountability
SCOPE assesses, reviews and supports agreed clinical work. The provider retains its governance and statutory responsibilities unless lawfully and expressly agreed otherwise.
Start a conversation
Outline the situation that needs attention. SCOPE can review the available information and clarify the appropriate clinical support.
