Care-plan creation and clinical review
Create, review or update care plans around the person’s current needs, risks and supports.
What does the care team need to know?
The work starts with the clinical information already available. SCOPE reviews the records together, identifies missing or inconsistent information and applies RN judgement to the clinical picture they form.
When this is useful
- A new care plan is required.
- A person’s needs, risks or supports have changed.
- An existing plan needs review or updating.
What the work can include.
Review available assessments, progress notes, medication and discharge information.
Clarify current needs, risks, supports and gaps in the available records.
Create or update the care plan, including monitoring and follow-up requirements.
What the provider receives.
A new or updated care plan that reflects the person’s current clinical position, with the information and follow-up the care team needs.
