Comprehensive RN assessments

Bring the findings together to understand the person’s current clinical situation.

What is happening clinically?

An assessment considers the findings together. Mobility, pain, cognition, medication information and the support available around a person can all influence the wider clinical picture.

When this is useful

  • A comprehensive RN assessment is required.
  • Changing needs require a current clinical view.
  • Assessment findings need to inform care planning or further review.

What the work can include.

  • Consider mobility, falls, skin integrity, continence, nutrition and hydration.

  • Review cognition, pain, medication information, function, daily living and current supports.

  • Use relevant validated assessment tools and Risk and Vulnerability Assessments where appropriate.

What the provider receives.

Clinical findings and RN interpretation that can support care planning, risk management, documentation, reassessment and further clinical action.
